1. That I am affiliating myself to the Council on my own volition and individual capacity;

2. That I am not representing any organization as a member of the Council;

3. That I whole-heartedly agree with the vision of the Council and shall support its objectives and activities with the best of my knowledge, skills, and abilities; and

4. That I shall abide with all the provisions of the Charter of the Council and the rules and regulations that shall be approved by the Council.
